STUDY THE FREQUENCY OF KLEBSIELLA PNEUMONIA AND SOME RESISTANCE GENES IN HOSPITAL ACQUIRED AND COMMUNITY ACQUIRED URINARY TRACT INFECTION PATIENTS

الملخص

Klebsiella pneumoniae is an important pathogen in nosocomial and community-acquired UTIs. It is showing a high resistance towards many drugs. This study aimed to estimate the percentage of K. pneumonia in Hospital acquired and Community acquired UTI, drug resistance, ESBL, and detect the resistance genes (bla KPC, bla CTXM, bla SHV) and presence of mutation in (bla KPC). A total of (200) urine samples were collected; K. pneumonia was cultured and isolated on MacConkey agar, antibiotic sensitivity testing and ESBL detection were performed by Vitek 2 system, then DNA was extracted, PCR and sequencing for KPC gene were done. There is 7% (14 isolates) of K. pneumonia (50% HAUTI and 50% CAUTI) within 21.43% positive ESBL were obtained. Percentage of positive bla SHV and bla CTXM genes were 100%, while positive bla KPC gene was 42.8%. KPC gene sequencing show high rate of mutation in comparison between positive isolates reach to 50 different nucleotides. This work has significantly increased the knowledge on the Klebsiella pneumonia causing HAUTI and CAUTI patients.
